We will verify the information you provide in your online request, and we might call to speak to you briefly to help ensure that we can offer you the services that you need. Following that, you will receive an email confirmation of your appointment.

For out-of-network billing, we will provide a “superbill” to the patient reflecting payment at the “Private Pay” rate for patient submission to their insurance company for reimbursement. The “superbill” contains all necessary diagnosis and treatment codes required by insurance companies. Please note that Larson Mental Health does not bill insurance companies for out-of-network claims. Please call if you have questions about your insurance, or if you would like to see your insurance company listed.
